During the last decades, shared decision making (SDM) has become a very popular model for the physician-patient relationship. SDM can refer to a process (making a decision in a shared way) and a product (making a shared decision). In the literature, by far most attention is devoted to the process. In this paper, I investigate the product, wondering what is involved by a medical decision being shared. I argue that the degree to which a decision to implement a medical alternative (...) is shared should be determined by taking into account six considerations: (i) how the physician and the patient rank that alternative, (ii) the individual preference scores the physician and the patient (would) assign to that alternative, (iii) the similarity of the preference scores, (iv) the similarity of the rankings, (v) the total concession size, and (vi) the similarity of the concession sizes. I explain why shared medical decisions are valuable, and sketch implications of the analysis for the physician-patient relationship. (shrink)

In this paper, we discuss Armstrong’s account of attachment-based claims to natural resources, the kind of rights that follow from attachment-based claims, and the limits we should impose on such claims. We hope to clarify how and why attachment matters in the discourse on resource rights by presenting three challenges to Armstrong’s theory. First, we question the normative basis for certain attachment claims, by trying to distinguish more clearly between different kinds of attachment and other kinds of claims. Second, we (...) highlight the need to supplement Armstrong’s account with a theory of how to weigh different attachment claims so as to establish the normative standing that different kinds of attachment claims should have. Third, we propose that sustainability must be a necessary requirement for making attachment claims to natural resources legitimate. Based on these three challenges and the solutions we propose, we argue that attachment claims are on the one hand narrower than Armstrong suggests, while on the other hand they can justify more far-reaching rights to control than Armstrong initially considers, because of the particular weight that certain attachment claims have. According to Peter Klein, foundationalism fails because it allows a vicious form of arbitrariness. The present article critically discusses his concept of arbitrariness. It argues that the condition Klein takes to be necessary and sufficient for an epistemic item to be arbitrary is neither necessary nor sufficient. It also argues that Klein's concept of arbitrariness is not a concept of something that is obviously vicious. Even if Klein succeeds in establishing that foundationalism allows what he regards as arbitrariness, this does (...) not yet mean that he confronts it with a sound objection. (shrink)

This paper evaluates Peter Klein’s objection to foundationalism. According to Klein, foundationalism fails because it allows arbitrariness “at the base.” I first explain that this objection can be interpreted in two ways: either as targeting dialectical foundationalism or as targeting epistemic foundationalism. I then clarify Klein’s concept of arbitrariness. An assertion or belief is assumed to be arbitrary if and only if it lacks a reason that is “objectively and subjectively available.” Drawing on this notion, I evaluate Klein’s objection. I (...) first argue that his objection construed as targeting dialectical foundationalism fails, since nothing prevents dialectical foundationalism from ruling out arbitrary assertions. I then argue that the objection seen as targeting epistemic foundationalism cannot be disqualified in the way some foundationalists believe. However, I show that also the objection so construed does not succeed, since epistemic foundationalism need not countenance arbitrary beliefs. (shrink)

The need for European citizens to be more involved in scientific research has emerged from the conclusions of the studies commissioned by the EC and by independent bodies. In the first part of this contribution, I will discuss the question of whether a dialogue between society and science is desirable. I will attempt to claim that at least one of the reasons why the dialogue between science and society should be defended has been underestimated in the course of most of (...) the studies carried out: the fact the scientific knowledge helps to a create a better, more rounded, more aware human being which can only benefit society. Subsequently, I will deal with the various genres of scientific literature. While in modern times, communication of scientific discoveries has been reduced to the article, in the nineteenth century scientists made use of more varied literary forms such as journals, letters and narrative essays which were more accessible to those who were not specialists but nevertheless curious about scientific matters. I believe that these more popular forms of writing helped to close the gap between the public and science. (shrink)

Most everyday tasks involve multiple modalities, which raises the question of how the processing of these modalities is coordinated by the cognitive system. In this paper, we focus on the coordination of visual attention and linguistic processing during speaking. Previous research has shown that objects in a visual scene are fixated before they are mentioned, leading us to hypothesize that the scan pattern of a participant can be used to predict what he or she will say. We test this hypothesis (...) using a data set of cued scene descriptions of photo-realistic scenes. We demonstrate that similar scan patterns are correlated with similar sentences, within and between visual scenes; and that this correlation holds for three phases of the language production process (target identification, sentence planning, and speaking). We also present a simple algorithm that uses scan patterns to accurately predict associated sentences by utilizing similarity-based retrieval. (shrink)

When people communicate, they coordinate a wide range of linguistic and non-linguistic behaviors. This process of coordination is called alignment, and it is assumed to be fundamental to successful communication. In this paper, we question this assumption and investigate whether disalignment is a more successful strategy in some cases. More specifically, we hypothesize that alignment correlates with task success only when communication is interactive. We present results from a spot-the-difference task in which dyads of interlocutors have to decide whether they (...) are viewing the same scene or not. Interactivity was manipulated in three conditions by increasing the amount of information shared between interlocutors. We use recurrence quantification analysis to measure the alignment between the scan-patterns of the interlocutors. We found that interlocutors who could not exchange feedback aligned their gaze more, and that increased gaze alignment correlated with decreased task success in this case. When feedback was possible, in contrast, interlocutors utilized it to better organize their joint search strategy by diversifying visual attention. This is evidenced by reduced overall alignment in the minimal feedback and full dialogue conditions. However, only the dyads engaged in a full dialogue increased their gaze alignment over time to achieve successful performances. These results suggest that alignment per se does not imply communicative success, as most models of dialogue assume. Rather, the effect of alignment depends on the type of alignment, on the goals of the task, and on the presence of feedback. (shrink)

The human sentence processor is able to make rapid predictions about upcoming linguistic input. For example, upon hearing the verb eat, anticipatory eye-movements are launched toward edible objects in a visual scene. However, the cognitive mechanisms that underlie anticipation remain to be elucidated in ecologically valid contexts. Previous research has, in fact, mainly used clip-art scenes and object arrays, raising the possibility that anticipatory eye-movements are limited to displays containing a small number of objects in a visually impoverished context. In (...) Experiment 1, we confirm that anticipation effects occur in real-world scenes and investigate the mechanisms that underlie such anticipation. In particular, we demonstrate that real-world scenes provide contextual information that anticipation can draw on: When the target object is not present in the scene, participants infer and fixate regions that are contextually appropriate. Experiment 2 investigates whether such contextual inference requires the co-presence of the scene, or whether memory representations can be utilized instead. The same real-world scenes as in Experiment 1 are presented to participants, but the scene disappears before the sentence is heard. We find that anticipation occurs even when the screen is blank, including when contextual inference is required. We conclude that anticipatory language processing is able to draw upon global scene representations to make contextual inferences. These findings are compatible with theories assuming contextual guidance, but posit a challenge for theories assuming object-based visual indices. (shrink)

In 1975, the English evolutionist William Donald Hamilton held in Brazil a series of lectures entitled “Population genetics and social behaviour”. The unpublished notes of these conferences—written by Hamilton and recently discovered at the British Library—offer an opportunity to reflect on some of the author’s ideas about evolution. The year of the conference is particularly significant, as it took place shortly after the applications of the Price equation with which Hamilton was able to build a model that included several levels (...) of selection. In this paper I mainly analyse the inaugural lecture in which Hamilton proposes a simple model to disprove the hypothesis supported by the British zoologist C. Vero Wynne-Edwards regarding mechanisms to prevent “over-exploitation of the food supply” in “the interests of the survival of the group”. The document presented here is of great historical interest. Not only because manuscript offers a model that—since it was intended for teaching purposes—had never before appeared in the published version, but also because of the general index of the lectures that accompanies it. The latter allows us to make some hypothetical considerations on the relationship and differences between kin-selection, group-selection and inclusive fitness that Hamilton wanted to present to the attentive, well-prepared audience of the foreign university that had invited him. AimsSport specialization is an actual trend in youth athletes, but it can increase injury risk. The aim was to determine the eventual correlation between sports specialization and injury risk in various sports, using a biopsychosocial approach.Methods169 sport-specialized athletes completed [; overall,, ] a self-reported questionnaire regarding sociodemographic, physical-attitudinal, injuries and psychological-attitudinal To analyze data univariate and correlate analyses were used.ResultsOf 169 athletes enrolled, 53% were single-sport specialized. In team sports a high risk of having to remain at rest for up (...) to 1 month because of overuse was observed. Males who suffered direct trauma in the team group were more exposed to that type of injury, and also the type of specialization figured significantly. In addition, interoceptive awareness correlates with injury. This study shows that, even though young athletes seem to be at a high risk of becoming injured, early team sport specialization and a high performance level cannot be considered to be the only risk factors. In this paper we present applications of the ACGT Master Ontology (MO) which is a new terminology resource for a transnational network providing data exchange in oncology, emphasizing the integration of both clinical and molecular data. The development of a new ontology was necessary due to problems with existing biomedical ontologies in oncology. In his essay From Logos to Mythos, I republished on the twentieth anniversary of its first publication, Most saw in the release of Creuzer’s Symbolik und Mythologie the moment when philology distanced itself from philosophy “refraining strictly from any discussion of what the Greek myths could mean for us, and instead concentrating […] into what the Greek myths could have meant for the Greeks”. This essay analyses the case study offered by the mythological dyad Kronos (Κρόνος) and Chronos (Χρόνος). It (...) highlights some elements that place these two figures at a distance on the philological level but closer on the philosophical one. The essay encourages a dialogue between philological rigour and philosophical use of myth: myth, in its prolificity, is eloquent in every age and a careful understanding of what it meant to the Greeks in its time does not necessarily imply giving up understanding what it may mean to us today. Before COVID-19, dementia singing groups and choirs flourished, providing activity, cognitive stimulation, and social support for thousands of people with dementia in the UK. Interactive music provides one of the most effective psychosocial interventions for people with dementia; it can allay agitation and promote wellbeing. Since COVID-19 has halted the delivery of in-person musical activities, it is important for the welfare of people with dementia and their carers to investigate what alternatives to live music making exist, how these alternatives are (...) delivered and how their accessibility can be expanded. This community case study examines recent practice in online music-making in response to COVID-19 restrictions for people with dementia and their supporters, focusing on a UK context. It documents current opportunities for digital music making, and assesses the barriers and facilitators to their delivery and accessibility. Online searches of video streaming sites and social media documented what music activities were available. Expert practitioners and providers collaborated on this study and supplied input about the sessions they had been delivering, the technological challenges and solutions they had found, and the responses of the participants. Recommendations for best practice were developed and refined in consultation with these collaborators. Over 50 examples of online music activities were identified. In addition to the challenges of digital inclusion and accessibility for some older people, delivering live music online has unique challenges due to audio latency and sound quality. It is necessary to adapt the session to the technology's limitations rather than expect to overcome these challenges. The recommendations highlight the importance of accessibility, digital safety and wellbeing of participants. They also suggest ways to optimize the quality of their musical experience. The pandemic has prompted innovative approaches to deliver activities and interventions in a digital format, and people with dementia and their carers have adapted rapidly. While online music is meeting a clear current need for social connection and cognitive stimulation, it also offers some advantages which remain relevant after COVID-19 restrictions are relaxed. The recommendations of this study are intended to be useful to musicians, dementia care practitioners, and researchers during the pandemic and beyond. (shrink)

No cure yet exists for devastating Alzheimer's disease (AD), despite many years and humongous efforts to find efficacious pharmacological treatments. So far, neither designing drugs to disaggregate amyloid plaques nor tackling solely inflammation turned out to be decisive. Mesenchymal stem cells (MSCs) and, in particular, extracellular vesicles (EVs) originating from them could be proposed as an alternative, strategic approach to attack the pathology. Indeed, MSC‐EVs—owing to their ability to deliver lipids/proteins/enzymes/microRNAs endowed with anti‐inflammatory, amyloid‐β degrading, and neurotrophic activities—may be exploited (...) as therapeutic tools to restore synaptic function, prevent neuronal death, and slow down memory impairment in AD. Herein the results presented in the most recently published studies on this topic are critically evaluated, providing a strong rationale for possible employment of MSC‐EVs in AD. (shrink)

The pandemic outbreak in March 2020 and its associated sanitary regulations and restrictions triggered an abrupt and significant change for society in general and for families’ organization in particular. In Chile, the Santiago Metropolitan District was under a strict lockdown that involved the closure of the entire educational system. From a systemic-family stress perspective, the impact of these changes might have consequences not only for each individual family member, but for the parental dynamic and, consequently, for children’s well-being. This paper (...) presents the results of a follow-up study showing changes in self-reported parental depression and the perceived home organization of mothers and fathers assessed at three different moments: before the pandemic, at the initial outbreak, and after 1 month of strict lockdown. Relevant moderators were explored using linear mixed models to understand the within-subject changes in mothers’ and fathers’ self-reports across the different assessment times. Financial strain, personality traits of self-criticism and dependency, previous parent–child quality interaction, recent major stressful events, and number of children are highlighted as relevant factors that moderate changes in home chaos and parental mental health perception. Significant risks and protective factors are described for fathers and mothers. The use of pre-pandemic measures as baseline levels enabled the identification of personal and family characteristics that were related to better outcomes. The results help increase our understanding of the sanitary regulations’ impacts on the family system and identify vulnerability indicators that should be considered. (shrink)

The implementation of online technologies to promote wellbeing is increasingly becoming a worldwide priority. This study includes secondary analyses of data and examined drop-out rates in an online guided self-help intervention for patients with anorexia nervosa. Specifically, rates of drop-out at end of treatment (i.e. six-week assessment), as well as intervention adherence (minimum of four of six online guided sessions) and differences between completers and drop-outs were examined. Motivation to change and associated patient variables were assessed as predictors of drop-out (...) using structural equation modelling. Ninety-nine patients were randomised to the intervention arm of the trial. Data were available for 82 individuals, 67 of whom completed the six-week assessment and attended a minimum of four online sessions. No significant differences were found between completers and drop-outs at baseline. At the end of the first week of participation, drop-outs from the six-week assessment or the intervention reported less satisfaction with their work with the mentor delivering online guidance. Greater confidence in own ability to change and higher controlled motivation (willingness to change due to pressure from others) predicted lower drop-out rates from the six-week assessment. Stronger alliance with the therapist at the treatment centre and lower psychological distress were associated with greater autonomous motivation (self-directed motivation) and importance and ability to change. Data demonstrate that a novel online guided self-help intervention for patients with anorexia nervosa is feasible. Early satisfaction with the program and external pressure to change have a protective role against drop-out rates. (shrink)

In this paper, we provide a logical formalization of the emotion triggering process and of its relationship with mental attitudes, as described in Ortony, Clore, and Collins’s theory. We argue that modal logics are particularly adapted to represent agents’ mental attitudes and to reason about them, and use a specific modal logic that we call Logic of Emotions in order to provide logical definitions of all but two of their 22 emotions. An unpublished satirical work, writtenc.1848–1854, provides fresh insight into the most famous scientific voyage of the nineteenth century. John Clunies Ross, settler of Cocos-Keeling – which HMSBeaglevisited in April 1836 – felt that Robert FitzRoy and Charles Darwin had ‘depreciated’ the atoll on which he and his family had settled a decade earlier. Producing a mock ‘supplement’ to a new edition of FitzRoy'sNarrative, Ross criticized their science and their casual appropriation of local knowledge. Ross's virtually unknown work is intriguing not (...) only for its glimpse of theBeaglevoyage, but also as a self-portrait of an imperial scientific reader. An experienced merchant seaman and trader–entrepreneur with decades of experience in the region, Ross had a very different perspective from that of FitzRoy or Darwin. Yet he shared many of their assumptions about the importance of natural knowledge, embracing it as part of his own imperial projects. Showing the global reach of print culture, he used editing and revision as satirical weapons, insisting on his right to participate as both reader and author in scientific debate. (shrink)

In our daily lives we conceive of our surroundings as an objectively given reality. The world is perceived through our senses, and ~hese provide us, so we believe, with a faithful image of the world. But occ~ipnally we are forced to realize that our senses deceive us, e. g., by illusions. For a while it was believed that the sensation of color is directly r~lated to the frequency of light waves, until E. Land (the inventor of the polaroid camera) showed (...) in detailed experiments that our perception of, say, a colored spot depends on the colors of its surrounding. On the other hand, we may experience hallucinations or dreams as real. Quite evidently, the relationship between the "world" and our "brain" is intricate. Another strange problem is the way in which we perceive time or the "Now". Psychophysical experiments tell us that the psychological "Now" is an extended period of time in the sense of physics. The situation was made still more puzzling when, in the nineteen-twenties, Heisenberg and others realized that, by observing processes in the microscopic world of electrons and other elementary particles, we strongly interfere with that world. The outcome of experiments - at least in general - can only be predicted statistically. What is the nature ofthis strange relationship between "object" and "observer"? This is another crucial problem of the inside-outside or endo-exo dichotomy. (shrink)
